What do I need for my first appointment?
When requesting an appointment, you’ll be sent information to access our patient platform, Modento. Once you create an account and log in, you can review and complete the new patient form. Please feel free to complete the form before your appointment. Additionally, please bring your insurance information (if applicable) and a list of medications you are currently taking.

We suggest having a cleaning and checkup every six months. But we may recommend more frequent checkups depending on your oral health needs.
