What to Expect in Your First Visit

To make your first appointment as convenient as possible, we’ve made our new patient paperwork available to complete online. When requesting an appointment, you’ll be sent information to access our patient platform, Modento. Once you create an account and log in, you can review and complete the new patient form. Please feel free to complete the form before your appointment.

In addition, please bring the following to your first visit:

• Your insurance information, including your insurance card
• If the insurance subscriber is a different person, the name and date of birth of that individual
• A list of any medications that the patient (you or a family member) is currently taking
